What Is DA Form 7801?

This is a military form that was released by the U.S. Department of the Army (DA) on July 1, 2019. The form, often mistakenly referred to as the DD Form 7801, is a military form used by and within the U.S. Army. As of today, no separate instructions for the form are provided by the DA.

How to Fill Out DA Form 7801?

DA Form 7801 instructions are as follows:

  1. Identify the soldier by their full name, rank, and the identification number assigned to them by the Department of Defense . Date the form and specify the lane and the firing order the person fired in. Enter the type of weapon used and the equipment that was attached to it if necessary. Check the box to confirm whether the scorecard reflects the results of practice or qualification.

  2. Use the first page of the document to describe the performance of the soldier during the day live-fire practice or qualification and fill out the second page of the instrument on behalf of the simulation’s center if the firer is trying to obtain the Chemical, Biological, Radiological and Nuclear certification or a night fire certification.

  3. List the results of the training event separated into four different categories that correspond to the firing positions the firer is supposed to assume - prone unsupported, prone supported, kneeling supported, and standing supported. It is required to assess the skills of the soldier in diverse situations - write down the total number of hits from various distances.

  4. Calculate the total number of hits to find out the final score of the firer . Assign an appropriate qualification rating to the soldier based on the results of the practice or qualification that took place. In case the following stages of the performance evaluation are approved, check the correct box.

  5. Assess the skill of the soldier to hit the target during additional stages that occur in different firing conditions - night fire and chemical environment. Specify their results and calculate the total number of hits.

  6. Use the remarks field to offer your opinion about the performance of the firer . This section of the form is completed by the experienced lane safety to share their insight regarding the ability of the firer. Once this is done, the officer in charge must state their full name and rank and certify the paperwork by signing it. The unit commander also authenticates the document that is maintained in the records of the unit for at least twelve months and is uploaded to the Digital Training Management System.
