What Is an Army RST Form?

Form 62-3R, Request for Authorization for Rescheduled Training (Rst) Equivalent Training (Et), or Excuse From Regular Scheduled Unit Training , is an official form issued by the Department of Defense (DOD) to be used by United States Army Reserve soldiers in the case they are unable to attend a scheduled Unit Training (UT). If such a situation occurs, a soldier has to file Form 62-3R.

Alternate Names:

  • Army RST Form;
  • Request for Rescheduled Training Form.

The purpose of the RST Form is to give a US Army Reserve soldier the option to reschedule a Battle Assembly/Multiple Unit Training Assemblies (MUTA) and for them to be able to make a request for a different date, alternate place or leave of absence (an authorized leave) if applicable, due to personal or medical reasons.

Form 62-3R was last revised on February 1, 1992 , with all previous versions are obsolete. An Army RST Form fillable version is available for download below.

How to Fill Out an Army RST Form?

A U.S. Army RST Form has to be filled out by a soldier together with their first line leader, whose responsibility will be to check the eligibility of a soldier's request. This form may be completed up to 60 days prior to the date in question.

Make-up dates and locations will be determined and recommendations will be noted by the first line leader. At this point the request will be submitted to the HHC (Headquarters and Headquarters Company) Commander, who will make a decision and file the bottom portion of the 62-3R Form, stating approval or disapproval. There are certain requirements listed in an RST Policy that soldiers need to meet to have an RST approved, such as personal records, health conditions, etc. It is strongly advised that only substantial causes be used to request an RST/Excused Absence to prevent abuse or misuse.

Form 62-3R is to be filled out by a soldier with a first-line leader prior to submitting a request to their Commander. Personal information needs to be provided - full name, Social Security Number, rank, section, and unit of assignment. Date(s) of absence and number of UTA's (Unit Training Assemblies) that will be missed needs to be provided. A corresponding box needs to be checked for Rescheduled Training (RST), Equivalent Training (ET), or Excuse from Regular Scheduled Unit Training (No make-ups).

Follow the instructions provided to understand which box to fill out next depending on the checkbox selected. Provide the reason for absence, sign, and date.
