What Is Form TE9?
Form TE9, Witness Statement - Unpaid Penalty Charge (Parking ), is a statement completed by the vehicle owner who wants to dispute the Order for Recovery they were provided after paying or appealing a parking ticket charge. The registered owner of the car might have paid the charge already, appealed it, made appropriate representations, or has not received any notifications urging them to deal with the outstanding debt. In such an event, additional penalties may be eliminated if you are able to prove you have complied with necessary regulations and due process.

How To Fill Out TE9 Form?
If you do not handle the Penalty Charge Notice within four weeks, you will be sent a Notice to Owner - this document obliges the offender to pay the parking ticket and gives them four more weeks to do it. The amount of money due will increase by 50% in case you ignore the new notice and you will receive a Charge Certificate. If you do not pay within two weeks, an Order for Recovery and Form TE9 will be mailed to your address. There are two options for you then: you either pay the penalty you owe or submit a Witness Statement to contest the allegations to the address indicated in the order. Do not ignore the documentation sent to you - otherwise, you may have to deal with enforcement agents and additional fees.
Form TE9 instructions are as follows:
- Enter the number of your penalty charge. Identify your vehicle by its registration number. Write down your name and the date and place of the parking offense.
- Indicate your title, full name, and address. If the vehicle in question belongs to the company, state the name of the business.
- Tick the box that correctly reflects the current situation. You can certify you have not received documents requesting you to pay the penalty, you have received no responses to your appeal or notices regarding the representations you have made, or you have already paid the penalty charge. In the latter case, you must record the date of payment, the payment method, and specify to whom you have paid the charge. Note that this list is limited, and you cannot refer to other valid excuses to explain why the charge remains unpaid.
- Confirm the declarations you have made in the form are true and complete. If you knowingly make a false statement, you may become subject to court proceedings and held in contempt of court.
- Write down your full name, sign and date the document. If you are preparing this form on behalf of your client or the individual you represent as a litigation friend, tick the appropriate box.
