What Is PS Form 1093-C?

PS Form 1093-C, Application for Post Office Caller Service is a document used to apply for a caller service, which permits a post office customer to obtain mail addressed to a PO Box number through a loading dock or a call window.

Caller service mail is provided separately for each paid caller service address. Reserve service lets customers reserve preferable caller addresses for a minimal fee. Users receiving large quantities of mail can use "locked bags" that have numbers like PO Boxes. This service is called Caller Service, and the assigned number is known as a Caller Number, although mail is generally addressed to a PO Box.

The Caller Service is a premium service accessible for a fee to a customer who:

  • Needs more than free carrier service;
  • Plans to receive an incoming volume of mail that cannot fit into the largest post office Box available;
  • Receives or intends to receive more mail that can be delivered to the largest post office Box available at the facility or wants to collect mail periodically during the day.

How to Apply for a Post Office Caller Service?

To apply for a Caller Service, the applicant has to complete the tear-off section on the third page of the form PS 1093-C and submit it to any postal service facility that offers window service. A falsified or incomplete application is a sufficient reason to discontinue or deny service. An application is not considered approved until the postal service confirms the applicant's identity.

The application for post office caller service contains the following bullets:

  • The purpose of the service - the business/organization or residential/personal use;
  • The name of the business/organization;
  • The full name of the person applying;
  • The full address;
  • The corporate address;
  • The telephone number;
  • The email address;
  • The quantity of numbers required;
  • The numbers for two items of valid identification;
  • The name(s) of all businesses or individuals, including members of a business, who will receive mail at this caller service;
  • The authorized representatives of the business/organization who will pick up mail;
  • The signature of the applicant and the actual date.

It is possible to pay Caller Service fees in advance for 6 months or 1 year. It is the caller's responsibility to pay the fee on schedule. If the fee is paid by mail, the payment has to be received by the due date. The approval date of the application determines the caller fee payment period.

Caller Service does not include a general delivery service. Customers who use Caller Service pick up their mail at the Post Office loading dock or call window several times a day as indicated by the postmaster.
