What Is PS Form 2976-E?

PS Form 2976-E, Customs Declaration Envelope CP 91 - also known as the USPS Form 2976-E - is a plastic envelope used to carry the United States Postal Service (USPS) customs forms and other necessary papers and protect them.

The latest version of the form was released by the USPS on July 1, 2013 with previous editions still valid. An up-to-date PS Form 2976-E fillable version is available for download below or can be found through the USPS website.

Customs forms contain the information about the sender, the addressee and a list of all items mailed in the parcel. They are required for all international mail and for parcels, send to Army Post Office (APO), Fleet Post Office (FPO) or Diplomatic Post Office (DPO) destinations. This is necessary for the border control of the accepting county. The forms are prepared in three copies: the sender keeps the first one, the second is for the customs and the third is kept at the post office for 30 days.

The PS 2976-E is designed to be transparent and keep all necessary data visible. This form is related to the PS Form 2976-ES, Small Customs Form Envelope, which is also a transparent envelope designed for customs forms, but it is smaller and made to fit into small parcels.

If a customs form is not necessary, the PS Form 2976-E is also not needed. A customs form is not necessary if the package contains only non-negotiable documents or correspondence, weighs under 16 ounces and is being sent with First-Class Mail International service.

When to Use PS Form 2976-E?

The form is required with any customs forms or when a PS Form 6182 (Commercial invoice) is used.

When a customs form is complete, put the two copies of the form and all other papers - such as the commercial invoice - into the envelope, remove the backing sheet and affix the PS Form 2976-E on the address side of the package. The envelope should not be folded or wrapped around the package.

  1. The PS Form 2976, Customs Declaration CN 22 (Sender's Declaration) is necessary for small packages sent via the First-Class Mail International, for First-Class Package International Service items, M-bags, and certain Priority Mail Express International items;
  2. All Priority Mail International shipments, except packages destined to military mail within the U.S.Territories, require the PS Form 2976-A, Customs Declaration and Dispatch Note CP 72;
  3. The PS Form 2976-B, Priority Mail Express International Shipping is used for all Priority Mail Express International Items and for Priority Mail Express items mailed to Army Post Office (APO), Fleet Post Office (FPO) or Diplomatic Post Office (DPO) destinations.
  4. The PS Form 2976-R, USPS Customs Declaration and Dispatch Note is used to describe packaged items and is used at border control.

How to Order PS Form 2976-E?

Use the USPS online store to order additional copies of the form. The envelope can be ordered in packages of 10 for free. The minimum order is 1 pack, that is 10 envelopes and the maximum amount is 50 Forms in 5 packs. The order will be delivered in 3-5 business days by Standard Post.

The PS Form 2976-E can be also obtained at a post office. All customs forms can be obtained there too, filled out and put into the envelope before mailing the parcel.
