What Is DA Form 4591-2?

DA Form 4591-2, Record of Inspections Conducted , is a form used for recording all inspections and Staff Assisted Visit (SAV) evaluations of the commander's Army Retention Program. The Army Retention Program is the program designed to dissuade highly qualified active-duty soldiers from separating from the Army and to help them re-enlist or transfer into the Reserve Component.

The latest version of the form - sometimes incorrectly referred to as DD Form 4591-2 - was released by the U.S. Department of the Army (DA) on March 1, 2016 . DA Form 4591-2 Instructions

Procedural guidelines and additional information can be found in Army Regulation 601-280. DA Form 4591-2 is used together with DA Form 4591, Retention Data Worksheet, and DA Form 4591-1, Status of the Retention Data Worksheets. The worksheets are completed and stored by the company commander or the retention Non-Commissioned Officer (NCO). The three forms are kept together in a three-ring binder, an accordion file, or in a filing cabinet.

DA Form 4591-2 consists of three sections with the three forms kept between separate section dividers.

  1. Section I ("Status of DA Form 459-1") is used by the company commander or the retention NCO to maintain copies of DA Form 4591-1.
  2. Section II ("Inspection Record") is used by the retention NCO for storing copies of DA Form 4591-2 together with a copy of all inspections reports and Staff-Assisted Visit (SAV) findings performed throughout the previous year. The commander is required to review and update the DA Form 4591-2 monthly. The form can be dated and initialed by the commander only after completing the monthly review.
  3. Section III ("Suspense File") consists of a set of additional dividers - the "Suspense Divider," seven yearly dividers, and a single set of twelve monthly dividers. This section is used for storing copies of DA Form 4591 for a single unit.
