DA Form 4591-1 Instructions
The DA 4591-1 is used together with the DA Form 4591 (Retention Data Worksheet) and the DA Form 4591-2 (Record of Inspections Conducted). All three forms are maintained by the company commander and company retention non-commissioned officer (NCO).
The worksheets are kept together in an accordion file, a three-ring binder or in a filing cabinet. The file consists of three sections with the three forms kept between separate section dividers.
- Section I - Status of DA Form 4591-1 - is where the company commander or the retention NCO will maintain copies of the DA Form 4591-1.
- Section II - Inspection Record - is the divider behind which the retention NCO will keep copies of the DA Form 4591-2 (Record of Inspections Conducted) along with a copy of all inspections reports and staff assisted visit (SAV) findings, performed during the last year.
- Section III - Suspense File - consists of a "Suspense divider," seven yearly dividers, and a single set of twelve monthly dividers used for storing soldier's DA Form 4591 copies.
