What Is PS Form 3811?
PS Form 3811, Domestic Return Receipt , is a document issued by the United States Postal Service (USPS) . It is used to request evidence of a specific mail piece delivery. The form was created by the PS to provide mailers with the legal confirmation of delivery or delivery attempt. It contains information on the exact delivery date and the person accepting the delivery.
The latest version of the receipt - also known as the USPS Form 3811 - was issued in on July 1, 2015 . The April version of the document remains in use until supply runs out. The actual receipts can be ordered at no charge from the PS official online store and found at your local Post Office.
It is not possible to purchase the Return Receipt service separately. It is available only in conjunction with the Certified Mail service, including the following domestic services:
- Priority Mail;
- Priority Mail Express;
- First-Class Packages;
- First-Class Mail.
If you did not receive your USPS return receipt, you can request the delivery information via the PS Form 3811-A, Request for Delivery Information/Return Receipt. The request should be submitted within 90 days from the date indicated on the mailing receipt. Besides, you will need to provide the receipt testifying that you have paid the return receipt fee.
PS Form 3811 Instructions
You can find the PS Form 3811 instructions in the top left corner of the document. The step-by-step completion instructions can be found below. You, as a sender, are required to complete items 1, 2, and 3. You also have to print legibly your name and address on the reverse side of the form. It is necessary for the PS could return you the receipt.
How to Fill Out PS Form 3811?
The USPS Form 3811 filling out instructions are as follows:
- You must indicate the mail piece delivery address in item 1. It should be domestic address since the International Return Receipt service is unavailable at present.
- Item 2 requires the Article Number. You can find it on the left edge of the certified mail receipt. Carefully remove the strip and stick it in the corresponding section of the USPS return receipt.
- Choose the proper service type from the list provided in item 3.
- Indicate your name and the address you want the receipt to be returned to in the corresponding box on the reverse of the form.
When the mail piece is delivered, the recipient has to sign the document and to provide the date of the delivery. The mail can be received by the addressee or a designated agent, like a secretary or a family member. If the latter is the fact, it should be indicated on the PS Form 3811. Moreover, if the delivery address differs from those, provided in item 1, the recipient will have to enter it in item D. Upon delivery, the completed form is removed from the envelope and returned to the sender with necessary information.
What Is the Article Number on PS Form 3811?
Еhe bottom left corner of the form contains an item titled "Article Number". It looks like a long string of digits and is unique for each mail piece. The Article Number on your return receipt and certified mail receipt must coincide.
Actually, this is your tracking number that allows you to confirm the fact of delivery online. For this purpose, you need to go to the USPS Tracking website, enter your number into the corresponding tracking number search box, and you will see the exact date and time that your mail was delivered. However, the information who have signed for the mail piece will not be displayed.
How to Attach PS Form 3811 to an Envelope?
You will notice two stripes on the reverse of the PS Form 3811 on both sides of the card. Neatly remove them and stick the form to your envelope or parcel. If using a regular envelope, you will need to fasten your USPS return receipt to the back of it. However, if you are sending a flat, both the certified mail and return receipts should be placed on its front part.
