How to Fill Out C1 Form?

1. If you have applied for help with court and tribunal fees, enter the "Help with Fees" reference number.
2. Specify the family court in question and name the children.
3. Provide your personal information (including your name, date of birth, relationship to the children, and contact details). If the document is prepared by the solicitor, they must state their information as well.
4. Record the names, dates of birth, and sexes of the children. Choose the order you need to enforce (for instance, if you want the court to decide where your child lives, request a Residence Order).
5. In case there have been other cases that involve your children, their siblings, or people who care for your children, indicate the court and provide contact information of all the parties.
6. Enter titles, names, addresses, and dates of birth of each respondent. Describe their relationship with the children.
7. If any other people must be notified about the order or there are other relatives or adults who live with the children, provide their personal information.
8. Write down the addresses of the children and describe their relationship to each other.
9. Check the box if you think the children may suffer from domestic violence.
10. If the children are known to the Social Services, name the social worker or provide details of the registration with the Child Protection Register.
11. Indicate the educational institutions the children attend and disclose whether they have any health issues or special needs.
12. Name the parents of the children, their relationship status, and whether they live together now. State your plans regarding the children and confirm whether you need an interpreter or special assistance for the hearing.
13. If you have separated from the other parent of the child, check the boxes to show whether you have adhered to the Parenting Plan.
