How to Fill Out C1A Form?

  1. Indicate your full name and gender. Record your status in the case by ticking the appropriate box. Add your telephone number. Confirm you have prepared Form C8, Confidential Contact Details, if you do not want the other party to know your address.
  2. List the names, dates of birth, and genders of the children. State their relationship to you.
  3. If you are represented by a solicitor, enter their name and name of the company they work for, contact information, and solicitor's reference.
  4. If you are the one making allegations, describe the nature of the abuse you or the children have experienced. Indicate whether it led to any involvement from the neutral parties like the police, doctors, and children's services. You need to tick the boxes to indicate the abuse you have been subjected to and list information about the court orders you have had.
  5. Provide brief descriptions of threatening behavior or violence you have suffered or witnessed. Later you will be able to make a long statement - for now only a short summary is enough. Follow the guidelines in the table to outline the details properly.
  6. In case you believe the children may be abducted, provide the reason for possible abduction and certify the threats or attempts to abduct. Write down the current location of the children and specify where their passports are at the moment.
  7. List other concerns you may have regarding the children.
  8. Record the orders or steps you want the court to make to help you and your children stay safe. Tick the boxes to allow or prohibit your children to spend time with the other party.
  9. Verify the statements in the form are true, write down your name, sign and date the document. If the solicitor fills out the statement, they must indicate the name of their firm and their position in it.
  10. Tick the boxes to request special arrangements you require to attend court safely.
  11. If you are the person who is being accused of threatening behavior, you must prepare a response to the allegations. Provide short comments on the statements you have read in the form, confirm this information is correct, sign and date the document.
