Form SUT 1 is used to record information about the details of a motor vehicle sale and purchase and to indicate the vehicle's specifications. It is strictly used for vehicle title. To complete the form, you will need to fill in the Vehicle Information section entering the year, make and body type of the car; sections for the date of the contract, names, and signatures of the seller and the buyer. Both parties of the transaction sign the appropriate Certification section.

If the car is five years old or less, you can use a generic Vehicle Bill of Sale. The document must be prepared to contain the basic required information with the seller's and buyer's signatures.

To certify the purchase price and to obtain a title for a motor vehicle five years old or less you are to use Form SUT 1A, Affidavit of Vehicle Purchase Price. This document provides information on the vehicle: its year, make and body type. It also contains the certification section, date of the purchase, applicant and the co-signer's name and signature. Note that Form SUT 1A has to be notarized.
