PS Form 3600-PM Instructions

The PS Form 3600-PM consists of thirteen pages in total. The first twelve pages are required to be filled out. The last page contains the instructions for filling out the form. Use either Commercial Plus or Commercial Base Prices. You should not combine both in one document.

The first page of the form - the cover sheet - contains the following mandatory fields:

1. Mailer Section. You must fill out this part completely, including the Permit Holder, the Mailing Agent (if any as described at page 13 of the form), and the Mail Owner (if other than the Permit Holder).
2. Mailing Section. Provide all the required details relating to the mail piece you are sending in this part.
3. Parts A through F and S. Complete those pertinent to your mailing.
4. Postage Section. Checkmark the applicable boxes, add all postages from the Part and Total Boxes, and enter the sum in Total Postage Box.
5. Certification Section. Read the disclaimer and sign the document.

Attach all filled out sheets to the first page and submit to the USPS.
