What Is a Field Trip Consent Form?

A Field Trip Consent Form is given to the legal guardian of a child, usually the parents, for them to fill in an agreement to take their child on a field trip - in most cases these are handed out by schools. The consent form will detail when and where the child will be taken along with any other important information that parents should be aware of, such as the method of transport.

Alternate Name:

  • Field Trip Consent Form to Parents.

When parents sign the Field Trip Consent Form, they agree to the risks associated with the trip and the form acts as confirmation that the school holds no liability. This of course doesn't mean your child will be in danger and that the teachers joining them on the field trip will not care for them or attempt to help them.

The job of the parent is to explain to their child the rules and conditions that come with the field trip. The parents must also inform the individuals accompanying their child about any known relevant medical conditions or medicine that the child would need to take. This would also be extremely useful in an emergency situation if the child has to be taken to a doctor. The accompanying adults are solely responsible for the medical aspect of the field trip.
