DD Form 368, Request for Conditional Release is a form used by members of National Guard or Reserve Duty soldiers who want to transition into positions of Active Duty.
The latest edition of the form was released by the Department of Defense (DoD) in August 2011 and is in use today with all previous editions being obsolete. An up-to-date fillable DD Form 368 is available for digital filing and download below or can be supplied through the Executive Services Directorate website.
The service member makes a request by filling out the form and submitting it to the officials from their chain of command to receive their approval. The DD 368 - in short - is a conditional release from the requestor's parent unit and branch of service. If the request is approved, the applicant is assigned to an Active Duty Army unit, which has an opening for the requestor's rank and MOS (Military Occupational Skill). In some cases, the soldier may get assigned to a military facility for different MOS training.
The DD 368 should not be confused with the Department of the Army-issued DA Form 638, Recommendation for Award.
