There are different reasons to prepare a Rebate Form:

  1. Rent Rebate. If you rent real estate for some time (even if it only happened for a week or month out of the entire year) and your income does not exceed the figure specified by state regulations, you can get a refund after property taxes are paid on the rented residence. In most cases, there are additional eligibility requirements - for instance, an applicant must be of old age or have a permanent disability;
  2. Purchase Rebate (also known as a "Customer Rebate"). A vendor can return a portion of the purchase price to a buyer. It usually happens when the purchaser bought goods of a specified value, quantity, or acquired them within a certain period of time. It is often filled out by clients of clothing stores;
  3. Sales Rebate (also known as a "Supplier Rebate"). You can request a rebate if you represent a retailer who wishes to receive payments from the manufacturer for each item sold to the customer. A Sales Rebate Form is commonly used in a variety of industries - a retailer is eligible for a rebate after vehicles and domestic appliances are sold.

How to Fill Out a Rebate Form?

Follow these steps to complete a Rebate Form:

  1. State your name, contact details, and identify yourself using a driver's license number, social security number, or federal employer identification number.
  2. Indicate the reason for requesting a rebate - you may write down the rent rebate amount, ask for a sales rebate from a manufacturer, or a purchase rebate from a retailer.
  3. If you are asking for a rent rebate, outline your income for the reporting period - salaries, lottery winnings, alimony, and inheritances you have received in the last year. Do not forget to submit proof of this income along with the form.
  4. Enter your savings or checking account details where the rebate amount will be sent.
  5. Print out the document and sign it. You need to present it in person or mail to the retailer that sold you items or the government agency responsible for the repayment, for instance, your state's Department of Revenue. Depending on the type of rebate, you must also submit photocopies of documentation that shows how many taxes you or the landlord had to pay or original proof of sale and purchase.
