"Teacher Self-evaluation Form"

A Teacher Self-evaluation Form is a document that can be used when a teacher would like to evaluate their work. The purpose of the document is to collect and present information about the teacher's self-assessment.

Generally, this type of form can be used in situations when a teacher would like to see if they have advanced their skills or if any of them need improvement. Teacher Self-evaluation is a significant process and in order for it to go properly, teachers should use a form that will contain all of the necessary information. If you prepare one yourself make certain it contains the following parts:

  1. Information About the Teacher. You can start your document by designating your name and the date when you did the self-assessment.
  2. Self-assessment Details. This part of the document can be divided into several sections, therefore, each section can use different types of questions to evaluate the teacher's skills. These questions can include choice questions (when a teacher is supposed to choose an answer from the offered variants), open-ended questions (when a teacher should formulate the answer in their own words), yes or no questions, and others.
