What Is a Change Order Form?
During a construction project, it is common for adjustments to be made to original estimates and scope, and these updates are recorded as a Change Order Form. This form will make note of any changes related to the project in terms of costs, labor needs, building plans, or building time length. A Change Order Form template can be downloaded below.
To create a Change Order Form include the following information:
- Name of the construction project.
- Location of the business or work site.
- The contact information for all construction managers, owner, and lead engineers.
- Name of the party requesting the change and date.
- Identification numbers for the contract and change order.
- Create a table listing the changes that need to be made, the reason for the changes, any supporting documentation, and specifications for each change.
- Include a detailed explanation for each item to clearly explain what parts of the project will need to be fully updated.
- Create a second table listing all items and materials that need to be purchased to complete the new requirements.
- A final table with the name and contact information for the person(s) requesting the updates.
- Signatures for all project heads.
