How to Fill Out a Peer Evaluation Form?

A Peer Evaluation Form for group work can be used for different kinds of projects, that will determine its content. Nevertheless, the document should contain parts, which include the following:

  1. Introduction . To avoid any misunderstandings the form should start with a title. It can be followed by some guidelines that will help individuals to evaluate their peers more effectively or recommendations on how to work with the form.
  2. Group Information . Enter information about their group, such as the evaluator's name, names of your peers, class period, etc.
  3. Project Data . Describe the project you have participated in. If filling in a Peer Evaluation Form for an oral presentation, you should designate the presentation topic or the purpose of the project and its main focus.
  4. Peer Evaluation . This part of the document is used for performance appraisals. State the name of the individual you are evaluating and describe their qualities using the questions presented in the document.
  5. Comments . The document should contain extra space for additional information that may later be found useful.

Depending on the type of project, the document can include different types of evaluating questions. It can be "yes" or "no" questions, open-ended questions, or tasks where the evaluator is supposed to rank their peers using a scale. For example, the evaluation part of the document can be presented as a table, where the evaluator should rate their peer's skills depending on their input, where 5 is "very good" and 1 is "poor." The evaluator should use this scale while answering questions such as:

  • How would you rank your peer's ability to contribute their ideas to the discussions?
  • Please rate the share of work that the peer has completed.
  • Please rank how reliable the peer is with the deadlines.
  • How would you rate the quality of work the peer has delivered?

Tasks like this one should be followed with open-ended questions, where the evaluator will have an opportunity to add more personality to the appraisal process. For example:

  • What is the individual's greatest strength?
  • Would you like to work with them again?

This will help form a better picture of the rated individual in order to evaluate them more successfully.
