What Is a DMV Change of Address Form?
A DMV Change of Address Form is a document that can be used by drivers when they would like to change the address on their driver’s license. The DMV has developed a separate address change form for each state and this document is used to inform the state government that a driver has relocated to another state and wishes to change the address as it appears on their license to match their new address. How to Change Your Address with the DMV?
